About the Practice
We want to ensure that your visit with us always exceeds your expectations. As such, we will work with you to make sure that you are seen in the timeliest manner possible. We look forward to working with you to make you an appointment that is as convenient as possible.
- OFFICE HOURS:
Monday through Friday from 9:30 am to 05:30 pm.
- Late arrival to appointments
Please make every effort to be on time for your appointment. We respect that you are taking time out of your busy schedule, and we strive to stay on schedule so that you don’t have to wait long.
Please call and let us know if you are going to be late to your appointment. We will try our best to minimize your wait and inconvenience. If you are late for more than 20 minutes, we will accommodate our schedule, however, you may be asked to wait or, on occasion, be asked to reschedule your appointment.
- Cancelling Appointments:
Your appointment is reserved for you; therefore, we ask if you need to cancel or reschedule to please call us at least 24 hours in advance. This will allow another patient to use that appointment time. Cancellations under 24 hours and/or No Show’s will be charged at $150.00. This fee is not billable to your insurance company.
- AFTER HOURS:
If you are having a medical emergency, please call 9-1-1 or go to the nearest emergency room.
For urgent matters, please call (818) 881-5661, and follow the prompts to leave a message on the urgent voice-mail box.
If due to technological difficulties you are unable to contact the office, please proceed to Providence Cedars Sinai Tarzana Medical Center.
We do not refill prescriptions after hours.
We will bill the insurance plans for which we have an agreement and will collect your estimated patient responsibility, (copay and/ or deductible), at the time of service.
Please contact your insurance company regarding your plan benefits and to confirm provider participation.
Please notify the office of any change in your coverage so that we can file the insurance claims in a timely manner. If the information is not provided and /or is not updated within 90 days of the time of service, the visit will be your responsibility.

If you do not have insurance coverage, payment is required at the time of service. We accept cash, Check, Visa and Mastercard.
Preventive visits are covered at 100 %. However, there are some plans which require a co-pay as well as some plans which do not cover Annual Exams. Please contact your insurance policy to confirm coverage.
As a courtesy a claim will be sent to your insurance company. Co-pays and estimated deductibles are due at the time of service.
Your OB package includes up to 13 prenatal visits, the delivery and the post-partum visit.
Our billing office will determine, if any, your covered benefits including any deductible, copayments, and/or coinsurance payments that you may owe for your obstetric services. This amount will need to be paid prior to receiving any benefits from your insurance company. Payment in full is due by the 28th week of your pregnancy, please make arrangements with the billing department to set up a payment plan that meets your needs. Upon delivery, your insurance company will be billed. Should you have overpaid, a refund check will be issued to you. If you have underpaid, we will bill you for the remaining balance.
All lab work, hospital charges, and any obstetrics services not provided by Dr. Danielzadeh will be billed by other entities.
Please note the following services are not included in the OB Package but are usually covered by your insurance (deductibles and copays apply).

- Obstetric ultrasounds
Drug prescription refills are only filled during regular business hours. Please contact your pharmacy to request a refill prior to contacting the office. We ask that you allow 48 hours’ notice for refill requests.
